How to use the caregiver and treatment schedule planner
Start with one typical week, including care duties and treatment time. The prescriber coordination planner can organize questions about communication with another provider. The outside therapist continuity tool can prepare a separate care-coordination discussion. Keep each concern in its own category so one answer does not imply another.
List fixed duties first. These may include work, school, meals, transportation, child care, or elder care. Then mark duties another trusted adult could cover.
Create separate columns for clinical fit, program timing, benefits, availability, and travel. A suitable schedule does not establish clinical fit or coverage. Likewise, confirmed benefits do not guarantee space or a start date.
For each open item, write who can answer it. Ask MVBH about its services and current scheduling. Ask the health plan about benefits, network status, authorization, and cost sharing.
